What kind of options exist for spinal pain. Traditional medications are OTC analgesics through to opioids, and in-between medications include those different from the usual. One of these would be gabapentin, which is a neuropathic pain medication. Gabapentin is used for epilepsy but off-label it is often prescribed for pain. Some antidepressants are also often used for neuropathic pain problems.
There are multiple issues that can go wrong with the spine. The soft cushion areas between the vertebral bones are called discs, and are like jelly donuts. They may experience little trears or bulges; at times these tears result in more extensive lumbar or cervical disc herniations and may push on nerve roots. This ends up with burning in the legs, called sciatica, or arm pain, called radiculopathy.
Another problem that may push on nerve roots is from arthritis and comes from soft tissue or bony overgrowth. This is called spinal stenosis and occurs in older individuals.
Spinal pain options include conventional medications such as OTC analgesics like NSAIDS through to opiates, and also in between medications. Another option for back and neck pain that works well is physical therapy. This includes strengthening of the core along with backa and neck muscles, bladder, and diaphragm. Additional treatments include spine decompression, which is a revolutionary treatment with intermittent traction providing nutrients to the disc. Ice, heat, electrical stimulation, laser, and acupuncture can also give significant relief.
Medications, PT, and alternative treatments may help tremendously. Also there are injections which can help avoid surgery and may be quite effective for back or neck pain. Studies nicely back up cortisone shots for pain relief to facet joints and if they work and then wear off a follow up procedure called radiofrequency ablation can be used. This can give over a year of pain relief.
Epidural injections work well. They are done under fluoroscopy, which is a real time x-ray. This helps locate the needle for more accurate positioning. The idea is to provide cortisone along with numbing medicine to irritate nerve roots and relax the swelling along with pain relief for sciatica and/or radiculopathy. Usually they are quite helpful.
There are 3 ways epidural injections are usually done. One is an intra-laminar approach, which is how the injections were done when epidural injections were first started. Another is a transforaminal approach, or a selective nerve root approach, where the medication is injected into the exiting tunnel in a more precise fashion. A third injection is called the caudal approach, which is above the coccyx in a small tunnel at the base of the sacrum.
